Community First Services and Supports (CFSS)

CFSS services is similar to PCA services. CFSS offers flexible options to meet the unique needs of individuals with disabilities, the elderly and others with special health care needs. CFSS services are provided in the individual’s home or in the community when normal life activities take them outside the home.

This program is part of a federally funded program that is administered under the state of Minnesota Department of Human Services, as well as the individual counties. CFSS services is composed of lead agencies, consultation providers, financial management services providers and CFSS provider agencies. 

Already receiving CFSS services through another agency? No problem.  We will complete all the paperwork with you in your home at a time convenient for you and will provide a seamless transfer of services to Caring Professionals Homecare.  Our UMPI provider numbers for CFSS services is: A609402000. 

Do you want to change providers and keep your CFSS worker?  We can help you change agencies and then hire, train and employ the CFSS worker as long as they meet the eligibility criteria, including the successful passing of a mandatory criminal background check.

Who do we accept payment from for individuals on Medical Assistance for PCA services?

• Medica SNBC or PMAP

We are unable to accept clients on Medica MSC+ or Medica MSHO at this time.
